| 400 | `invalid_json`, `invalid_request` (with per-field `details`) |
| 401 | `unauthorized` |
| 404 | `not_found` |
| 409 | `no_policy`, `not_open`, `not_in_review`, `closed` |
| 413, 415 | `file_too_large`, `unsupported_type` |
| 422 | `duplicate_external_reference`, `claim_too_large`, `idempotency_conflict` |
